Step 1
Get the Pledge Form
Download the Charity Pledge Form.
The PDF may require Adobe Acrobat Reader, download it here.

Step 2
Choose a charity or charities to donate to.
View Local (PDF Page 95-101) and National charities using the 2007 PDF Book available here: 2007 INWCFC Charity Listings Handbook

Step 3
Complete the form.
Copy the 5-digit Charity ID Code (or codes, if you are contributing to multiple charities) to your form.

Step 4
Sign and Return your Form.
After you complete the form make sure you sign it. Your signature is required.
Give Page 1 and 2 to your Campaign Coordinator, and keep Page 3 for your tax records.
